Welcome to North Jersey Eye Associates.  Here is some important information to get you started.   We treat patients of all ages, from children to seniors.  We participate with medicare and most insurance health plans. However please discuss your specific coverage with our staff.  For patients with HMO insurance  be aware that many of these plans require a referral from your primary care provider.  Without this referral you may be personally responsible for all charges or need to reschedule your appointment.

The initial exam is comprehensive and may take up to one hour.  Your eyes may be dilated and it could be difficult to drive afterward.  You should bring sunglasses and consider having a driver take you home.  

Below are the forms which you can download and complete prior to your visit in order to facilitate registration.

Please printout both patient Information and Medical History forms.

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's web site and enable you to download the Acrobat program to your computer.

Please bring ALL of your insurance cards and your referral if required. Bring a list of all your current medications. If you wear glasses please bring them even if they are broken or unsatisfactory. If you wear contact lenses, you may wear them to the office, (bring any contact lens information or old boxes) but please also bring your glasses.
